The following table shows how many completed surveys you must return for each average monthly script volume range. The third column in the table shows you how many questionnaires PatientDynamics will send you if you have this monthly script volume. This number is more than the number of completed questionnaires you must return, as in our experience, there are generally cases of mistakes being made on questionnaires and questionnaires being taken by patients and not returned.
